Textbook Condition Contract

Once you receive your textbooks, fill out this form to report the condition of your textbooks. If this is not filled out, you may be responsible for any damage at the end of the year.

This form is to verify damage from textbooks you received at our registration checkout. Any damage not reported below, you may be held liable for and you will be responsible for paying for repairs or replacement copies at the end of the school year. Please check over your books carefully, cover your books while keeping them at home, but do NOT use adhesive covers of any kind.

Note: Not everyone received the same textbooks, depending on your grade level and classes you got placed into. Please make sure you received two textbooks for 7th grade or one textbook for 8th grade at registration. If you feel you got the wrong textbooks, or if you have any questions, please reach out to our librarian at [email protected]

Should my book be damaged in such a way that it cannot be reissued to another student (i.e written obscenities that cannot be erased, major water damage, etc), I understand that I will be charged the replacement cost of the book.
